I normally require all potential clients to complete an intake questionnaire. There are
several reasons why I require this. After 17 years of law practice in two states, I have
represented many, many clients. I am required by ethics rules to do a conflict check on
all potential clients in order to ensure that that you and your legal matter do not present
a conflict of interest with any prior clients that I have represented. Furthermore, both your
time and my time are valuable; there is no sense in wasting each other’s time if it is
clear that your matter is something that I will not be assisting you with. Finally, if your
matter appears to be something I might be able to help you with, having certain basic
information on hand at the outset will make our consultation more productive. If you are
interested in having me take a look at your situation, please take a moment to complete
and then email me for an intake questionnaire

Employers:
I am required by ethics rules to do a conflicts check on all potential clients in order to
ensure my representation of past or present clients does not create a conflict of interest
in representing your organization. Therefore, before I can get into the details of your
organization’s legal matter, it may be necessary for me to obtain some basic information
about your organization. This preliminary information can be gathered easily, usually in
only a few minutes, and will allow me to determine whether I can go forward with
representation free from actual or potential conflicts.

After we agree that I will represent you or your organization in a legal matter, and sign
a written fee agreement, I will begin the process of creating a “file” containing the
documents relating to your legal matter. In the early stages of my representation or in
instances where your legal matter will generate relatively few documents, they will likely
be stored in paper form at my office and/or the hard drive of my computer. If your legal
matter becomes document intensive (usually the case if litigation occurs), I will transfer
your legal matter to my business server, where all of your documents will be stored. When
that happens, I will provide you an internet address and password that will allow you to
log onto my server, at any time, to view your documents, add/create new documents, and
to send me secure messages. If you lose the server web address or password, please call
me as soon as possible so that I can terminate your old password and issue you a new one.
